Required Forms for Top Talent
Forms listed below are required for you to review. You and parents need to be fully aware of all rules and regulations to apply for Top Talent LIVE 2025. All contestants for the Top Talent LIVE 2025 event must complete the contestant waiver. Be sure to complete it and obtain required signature.
Failure to submit ALL required documents by February 21, 2025, may result in disqualification.
For your convenience, download the forms listed below to your desktop, complete the documents and send a professional head-shot by February 21, 2025 to TopTalent@ocps.net.
We look forward to working with you. All the names of top finalists will be listed on our website March 28, 2025. If you are selected as a finalist, the LIVE show will take place May 3, 2025. We will provide all details to finalists' once selections have been made. For now, save this date on your calendar.
